Client Referral Information
Referring agencies are vital partners who help the Furniture Bank turn empty houses into homes. Agency case managers assist by qualifying and sponsoring a client, completing the paperwork, and sometimes paying the delivery fees. The case manager always accompanies her/his client to the Furniture Bank for their furniture selection appointment - to guide their client in selecting the 13-15 pieces of furniture needed most for the client's home and family.

Forms and Documents for Case Managers and Sponsors to Use
The Client Referral Form, Client Demographic Questionnaire and Client Furniture Needs forms are available as fillable forms that can be completed online, saved on your computer, and printed prior to your arrival at the Furniture Bank. Please complete the Client Referral Form (Exhibit F) in advance.
Periodic One-At-A-Time Referrals.
As often as possible, when the furniture supply is adequate, the Furniture Bank is able to accept one-at-a-time referrals from smaller agencies, churches, employers, civic groups, etc. If you know a family in need, you can sponsor them for a FREE furniture selection appointment. New Caseworker Orientation.
